Personal Advisor / Support Worker – Children with Disabilities Team – Berkshire - £16/h

Are you an experienced personal advisor looking to work in an environment where you can truly make a difference? Frontline are recruiting for a Personal Advisor / Support Worker to join a specialist children with disabilities team in Berkshire. You will be working alongside committed social workers to help care leavers with disabilities (between 18-25s) in reviewing and implementing their EHC plans.

The role is commutable from Berkshire, Buckinghamshire, West London and Surrey, with strong transport links via the M25 and M4.

The Personal Leaving Care Advisor role supports care leavers aged 16-25 years in their transition to adulthood. The role includes the completion, monitoring and reviewing pathway plans that provide focused outcomes for young people with disabilities.

Duties include:

  • To co-ordinating and assessing the impact of pathway plans
  • Undertaking planned work with young people and their support networks to promote their achievements and transition to adulthood.
  • Working with young people, families, carers and communities to help them make informed decisions, enabling them to take an active role in making plans and achieving outcomes in line with their needs.
